Insurance Reimbursement Instructions
We are proud to announce that after receiving our National Provider Identification number we are now considered to be an out-of-network Clinical Soft Tissue Therapy Provider. This means that insurance reimbursement for our services is now possible. This is dependent upon your individual insurance plan but we have already had success with clients receiving reimbursement and pre-approval for reimbursement benefits. Please read our directions carefully as we do not submit claims directly to your insurance company and there are several steps you must take to receive reimbursement.
Directions for Using Your Insurance Benefits for Clinical Soft Tissue Therapy
How it works:
- First, it is important to call your insurance company to verify your benefits. Be sure to write down the information you receive, the name of the person you speak to and the date. Here are the questions you should ask your insurer:

- Does my plan have out-of-network benefits for manual therapy or massage therapy?
- Are CPT codes 97124 and/or 97140 covered?
- Do I need a referral from my doctor or physical therapist?
- Is pre-authorization required?
- What is my deductible and does that need to be met before out-of-network benefits can be received?
- What is my reimbursement rate?
- If you need a referral for reimbursement, visit your doctor to receive one. We have made a referral template you can bring with you to make sure all the pertinent information the insurance company needs is covered. That template and a list of relevant diagnosis ICD-10 codes is attached to this email. We recommend printing this out double sided to take with you to your medical appointment.
- Book and attend your massage therapy appointment. Please bring a copy of your referral with you.
- Pay after your appointment as usual. Sessions are billed at our standard clinical rates.
- We will provide you with a detailed receipt called a superbill after your session.
- Submit your superbill to your insurance provider through your online portal to receive reimbursement.
What’s included on your superbill
- Our provider information, including our National Provider Identification number (NPI)
- Date of service
- CPT code 97140 – Manual therapy techniques (soft tissue mobilization/manipulation, manual lymphatic drainage, manual traction, etc.) or CPT code 97124 – Therapeutic procedure for massage therapy ( effleurage, petrissage,tapotement, etc)
- Session length
- Fees paid
- Relevant ICD-10 diagnosis codes
- Clinical notes
- Therapist signature
Important: Packages vs. Insurance
Our discounted packages are best suited for self-pay and HSA/FSA clients. If you plan to submit to insurance, we recommend paying per visit rather than purchasing a package. This is because insurance reimburses per session, not in prepaid bundles, so per-visit billing creates clean and easy-to-process claims.
HSA / FSA
We continue to gladly accept HSA and FSA cards for all services. HSA/FSA can also be used to purchase our discounted packages.
